Conditions of Volunteer Placement (Please Read Carefully)

• I agree to comply with PVHS policies and procedures.
• I will be punctual and carry out my duties to the best of my abilities.
• If, for any reason, I am unable to attend a scheduled volunteer shift, I will promptly notify the Shelter Manager and refer to
  the Volunteer Contact List to find a replacement for myself. I will notify the Shelter Manager of who will be replacing me
  OR that I have made every effort to secure a replacement, but have been unsuccessful. This information will be provided in a
  timely manner to cause minimal disruption to the shift.
• I agree to hold in strict confidence all confidential information which may come to my knowledge in regards to other
  volunteers, staff, directors, adopters, donors, finances, animal intakes, animal adoptions, and animal health. I cannot share or
  release any confidential information without signed authorization from a PVHS Board Member.
• I acknowledge that I will be performing volunteer tasks for the Pembina Valley Humane Society that will involve, or be
  located near, animals and that there are risks associated with such volunteer activities.
• All of the information contained in this application is true and the misrepresentation of any part of this application will be just
  and sufficient cause for termination of my volunteer placement.
• I understand that I am not an employee of Pembina Valley Humane Society and that any duties that I perform are as a
  volunteer. I agree to abide by the procedures set forth by Pembina Valley Humane Society for my assigned work duties. I
  also understand that it is my responsibility to update any address, emergency or other changes to the information on this
  form.
• I, hereby release Pembina Valley Humane Society, its staff, Board, donors, sponsors and volunteers from all claims in respect
  to death, injury, loss or damage to my person or property arising from my participation in activities or programs as an active
  participant or spectator.

Please note: Submitting a volunteer application does not guarantee acceptance in the PVHS volunteer program. Processing
applications takes time. Please be patient.
